When to see an Upper GI surgeon
If you are experiencing any of the following, a consultation with one of our upper GI surgeons may be appropriate:
- Persistent acid reflux or GORD not adequately controlled by medication
- A diagnosed or suspected hiatal hernia
- Difficulty swallowing (dysphagia) or chest pain related to the oesophagus
- Achalasia or another oesophageal motility disorder
- Gallstones cause recurrent pain, inflammation, or complications
- A diagnosis of oesophageal cancer or Barrett’s oesophagus requiring surgical assessment
- Previous upper GI surgery has not delivered the results expected
A GP referral is required to book your initial consultation. If you are unsure whether surgery is the right next step, we will take the time to review your history and recommend the most appropriate course of action, which may or may not involve an operation.

Achalasia Treatment
Achalasia is a condition where the lower oesophageal sphincter fails to relax properly, preventing food and liquids from passing freely into the stomach.
Treatment involves cutting the tight muscle at the base of the oesophagus, a procedure known as myotomy, to restore normal swallowing.
This can be performed laparoscopically (Heller myotomy) or via an endoscopic approach (POEM). Most patients experience significant, lasting relief following treatment.

Anti-Reflux Surgery
Fundoplication is the gold-standard surgical treatment for chronic acid reflux (GORD) and hiatal hernia.
The upper portion of the stomach is wrapped around the lower oesophagus to reinforce the valve between the stomach and oesophagus, preventing acid from rising into the throat.
The procedure is performed laparoscopically in most cases, resulting in a shorter hospital stay and a faster recovery. It is typically recommended for patients whose reflux is not adequately controlled with medication alone.

Oesophagus Removal
Oesophagectomy is the surgical removal of part or all of the oesophagus, most commonly performed to treat oesophageal cancer or severe pre-cancerous conditions such as Barrett’s oesophagus with high-grade dysplasia.
The stomach or a section of the colon is then repositioned to restore the continuity of the digestive tract.
It is a complex procedure requiring specialist expertise and is performed at an accredited Melbourne hospital with full multidisciplinary support.

Gallbladder Removal
Cholecystectomy is one of the most commonly performed surgical procedures in Australia and is the definitive treatment for gallstones, chronic cholecystitis (gallbladder inflammation), and gallbladder dysfunction.
The procedure is performed laparoscopically in almost all cases, making it a day procedure or a short overnight stay for most patients.
Recovery is typically straightforward, with most patients returning to normal activities within 1-2 weeks.

Hiatal Hernia Repair
A hiatal hernia occurs when part of the stomach protrudes through the diaphragm into the chest, which can cause chronic reflux, chest discomfort, and difficulty swallowing.
Surgical repair involves returning the stomach to its correct position and tightening the opening in the diaphragm to prevent recurrence.
In many cases, the repair is combined with a fundoplication to address reflux simultaneously. Most repairs are performed laparoscopically with a short hospital stay.
Your Upper GI surgery journey
Understanding what to expect at each stage helps reduce uncertainty and lets you feel fully prepared. Here is what the process typically looks like at Specialist Surgical Group.
1. GP referral
Your GP refers you to one of our upper GI surgeons. A standard referral is valid for 12 months and entitles you to a Medicare rebate on your consultation fee.
Ask your GP to address the referral to a named surgeon where possible.
If you are unsure which surgeon is right for your condition, we can help guide you when you call.
2. Initial consultation
You will meet with us to discuss your symptoms, medical history, and any relevant test results or imaging.
We will explain your diagnosis and treatment options, answer your questions, and begin forming a personalised plan.
In some cases, additional investigations such as an endoscopy, CT scan, or oesophageal manometry may be requested before a final recommendation is made.
3. Pre-operative assessment
Before surgery, you will complete a pre-operative workup that may include blood tests, imaging, an ECG, and any specialist assessments relevant to your procedure.
This ensures you are in the best possible condition for surgery and allows us to finalise the operative plan.
You will also receive clear instructions on fasting requirements, which medications to pause or continue, and what to bring on the day.
4. Procedure day
All upper GI procedures are performed at an accredited Melbourne hospital by a dedicated theatre team and us. Most patients are admitted on the morning of surgery. Hospital stay varies by procedure.
A laparoscopic cholecystectomy is typically a same-day or overnight admission, while a more complex operation, such as an oesophagectomy, requires several days.
5. Post-operative recovery
We will provide written and verbal guidance on your post-operative diet, activity restrictions, wound care, and warning signs to watch for during your recovery at home.
A follow-up appointment is scheduled before you are discharged. You can contact our rooms directly during business hours with any questions, and after-hours support is available through your treating hospital for anything requiring urgent attention.
6. Long-term follow-up
Your care continues well beyond surgery. We schedule follow-up appointments at 2 weeks, 6 weeks, and 3 months post-procedure, with ongoing access to our care team for as long as you need support.
For more complex procedures such as oesophagectomy, follow-up schedules are tailored individually and may include specialist imaging or oncology coordination as part of your long-term care plan.

Dr Ian Michell, MBBS, FRACS
Bariatric, Gastrointestinal & General Surgeon
Why choose us
Our difference lies in combining highly experienced surgeons with the latest surgical innovations and individualised care that puts you first.

Experienced surgeons
Dr Chek Tog, Dr Ben Keong, Dr Ernest Lim, and Dr Ian Michell are all Fellows of the Royal Australasian College of Surgeons (FRACS) with advanced specialist training in upper gastrointestinal surgery.
They trained at leading Australian institutions and have collectively performed thousands of upper GI procedures at accredited hospitals across Melbourne, bringing specialist expertise to conditions ranging from GORD to oesophageal cancer.

Minimally invasive techniques
Wherever clinically appropriate, our surgeons use laparoscopic (keyhole) techniques that involve just a few small incisions.
Compared with open surgery, this approach typically results in less postoperative pain, a shorter hospital stay, a reduced risk of wound complications, and a faster return to normal activities.
For complex cases such as oesophagectomy, a hybrid or open approach may be required and will be discussed with you beforehand.

Multi-disciplinary team
Our surgeons work alongside dedicated nurses, dietitians, and care coordinators at every stage of your treatment from pre-operative assessment through to long-term follow-up.
This integrated approach ensures that surgical, nutritional, and allied health needs are addressed together rather than in isolation.
For patients undergoing complex procedures, we also coordinate with oncologists and other specialists as required.
Our reputation speaks volumes

Dr Chek Tog, Dr Ben Keong, Dr Ernest Lim and Dr Ian Michell are all Fellows of the Royal Australasian College of Surgeons (FRACS) with advanced specialist training in upper gastrointestinal surgery. They have collectively performed thousands of procedures at accredited hospitals in Melbourne.
Yes. A referral from your GP or specialist is required. A standard GP referral is valid for 12 months and entitles you to a Medicare rebate on your consultation fee. Ask your GP to address the referral to one of our upper GI surgeons by name.
Most upper GI surgical procedures are covered under private health insurance if you hold an appropriate level of hospital cover and have served any applicable waiting periods (typically 12 months for pre-existing conditions). We recommend contacting your insurer before your consultation to confirm your entitlements. Medicare rebates apply to the surgeon’s fee regardless of your insurance status.
In most cases, yes. Our surgeons perform the majority of upper GI procedures using minimally invasive laparoscopic (keyhole) techniques. This typically results in a shorter hospital stay, less post-operative discomfort, and a faster return to normal activities. In some complex cases, such as oesophagectomy, open or hybrid approaches may be required, and we will discuss this with you at your consultation.
Surgery may be appropriate if your reflux symptoms are not adequately controlled by proton pump inhibitors (PPIs) or other medications, if you prefer a long-term solution over ongoing medication use, or if a structural cause such as a hiatal hernia has been identified. We will review your history and any prior investigation results to help determine whether surgery is the right option for you.
A hiatal hernia repair involves returning the stomach to its correct position below the diaphragm and closing the hiatal opening. Fundoplication is an anti-reflux procedure that wraps the upper stomach around the lower oesophagus to strengthen the valve. The two procedures are often performed together, as hiatal hernia and acid reflux frequently coexist.
Recovery varies by procedure. For laparoscopic cholecystectomy, most patients return to normal activities within one to two weeks. After fundoplication or hiatal hernia repair, most patients resume light activity within 2 weeks and normal activity within 4-6 weeks. We will provide a tailored recovery plan at your pre-operative appointment.
Our upper GI surgeons treat conditions including chronic acid reflux (GORD), hiatal hernia, achalasia, gallstones and gallbladder disease, Barrett’s oesophagus, and oesophageal cancer. If you are unsure whether your condition requires a surgical assessment, contact us and we will be happy to advise.
